Three (3) Sizes of Add-on Forms
- Small Forms:
- Contains from 1 to 9 input fields
- Medium Forms:
- Contains from 10 to 20 input fields
- Large Forms:
- Contains from 21 to 40 input fields
- –additional per-field fee for more than 40 fields
Provide the title of your form, field labels and types. Include a sketch of your form to specify the order of your fields.
